| Members and their covered spouses enrolled in Anthem or United Healthcare Commercial (non-Medicare Advantage) Plan Options are each eligible to earn up to 480 points by completing Be Well SHBP program activities. Members and their covered spouses can redeem points for 2025 reward. •A $150 Sharecare Reward Visa Prepaid Card (can be used anywhere Visa is accepted. It takes up to eight weeks to receive the card after redemption and expires 12 months after the issuance date) OR •480 well-being incentive credits (to apply toward eligible medical or pharmacy expenses.
