Hospital/Homebound (HHB) Instruction

Hospital/Homebound (HHB) instruction provides temporary educational support for Clayton County Public Schools (CCPS) students whose medical conditions—physical or psychiatric—prevent them from attending school. HHB services help maintain continuity of learning between the school, home, and healthcare settings when a student is unable to attend school for at least 10 consecutive or intermittent school days, or 5 days on a block schedule, during the school year.

HHB services require completed forms certified by the licensed physician or psychiatrist currently treating the student. Hospital/Homebound instruction is a short-term educational support and is not intended to replace regular school services. While the goal of HHB is to maintain continuity of instruction and support a successful transition back to school, academic progress during HHB instruction cannot be guaranteed.

Eligible Students

Students may qualify for Hospital/Homebound services when they meet the following criteria:

• Currently enrolled in Clayton County Public Schools
• Expected to be absent for 10 or more consecutive school days
• Have a chronic illness that may require intermittent HHB services, as certified by a licensed physician or psychiatrist
  • Additional medical documentation may be required for intermittent services
• Have a medical condition that is not contagious
• Are medically able to benefit from instructional services

Students Who Are Not Eligible

Students are not eligible for HHB services if they:

• Attend school for a partial or half day
• Have a communicable disease
• Are receiving instructional services through other educational sources

Referral Process

The Hospital/Homebound referral process includes the following steps:

1. A completed Hospital/Homebound Referral Form must be submitted.
2. If approved, certified teacher(s) will be assigned to the student.
3. The assigned teacher will coordinate with the family to establish a mutually agreed-upon date, time, and location for instruction.
4. If the student does not return to school as originally projected, a medical extension must be submitted to continue services.

Parent/Guardian Responsibilities

Parents and guardians play an important role in supporting the HHB process. They are responsible for:

• Obtaining the required Hospital/Homebound referral forms
• Ensuring the physician or psychiatrist currently treating the student completes and signs the Medical Certification Referral Form
• Returning all completed forms to the school or Hospital/Homebound Department
• Notifying the HHB teacher(s) of cancellations or necessary changes to scheduled instruction times

Instruction

Hospital/Homebound instruction may be provided based on the student's individual medical and educational needs.

• A student is counted present when receiving a minimum of three hours of instruction per week
• Instruction may be provided in the home, hospital, or through telecommunication
• A responsible adult must be present in the home for the duration of instruction
• An IEP will be developed or amended for students with disabilities, as appropriate
• The amount of instruction provided is determined by several factors, including the severity of the student's medical condition

Grades

Grades are determined collaboratively by the student's classroom teacher(s) and Hospital/Homebound teacher(s).
