CCPS Kinshop Caregiver Form

Kinship care refers to an arrangement where an adult is raising a child whose parents cannot provide care for their minor child. The adult providing care, whether relative or non-relative, has a relationship or bond with the child and the arrangement may be temporary or permanent.  It commonly refers to relatives raising children who are not in foster care. The Kinship Caregiver Affidavit should not be used by a biological parent whose name does not appear on the minor child's birth certificate.

To ensure the continued safety of all children, enrolling adults utilizing the Kinship Caregiver Affidavit will be required to provide supporting documentation to submit along with the document. The supporting documents will be reviewed at the time of enrollment. It is very important to note that the Kinship Caregiver Affidavit cannot be utilized for athletics.  The affidavit cannot be utilized for a student to reside in a school's attendance zone for football, basketball, volleyball, soccer, track, or cheerleading. Additionally, the affidavit cannot be used for marching band.

A kinship caregiver is authorized to enroll a child in school by completing a Kinship Caregiver Affidavit.  The affidavit will expire at the end of the school year for which it was used. The affidavit does not carry over to the next school year.

The kinship caregiver will serve as the school’s point of contact for the child regarding truancy, discipline, and educational progress, for one school year.
