What You'll Need to Register
Starting Registration is Easy!
You can begin registration even if your child’s health records are not yet up to date.
Required immunizations and health screenings should be completed before school begins on August 3, 2026.
Required to Submit Registration
Have these documents ready before submitting your online registration:
☐ Proof of Student Age
Birth certificate, passport, or another official document showing the student’s date of birth.
☐ Two Proofs of Residency
A current utility bill and one additional approved residency document showing the same home address.
☐ Parent/Guardian Photo ID
Driver’s license, state-issued ID, passport, or other government-issued photo identification.
☐ Proof of Authorization to Enroll
Required if the person registering the student is not the student’s birth or adoptive parent.
☐ Georgia Certificate of Immunization — Form 3231

☐ Vision, Hearing, Dental & Nutrition Screening — Form 3300
Screenings should be current and completed within the past 12 months. Click on the button below for more information. Georgia Health Screening Requirements
If Your Child Attended Another School
You may also need:
☐ Report card or transcript
☐ Previous school withdrawal form
☐ Discipline records, if applicable
☐ Other educational records requested by the school
Optional Information
☐ Student Social Security Number
Families will be asked to provide the student’s Social Security number, but a student may still be enrolled if it is not provided.
